Key points
- Linagliptin blocks DPP-4, the enzyme that breaks down the gut hormones that raise insulin after a meal, so insulin rises only when blood sugar is high.
- It leaves the body mainly through bile rather than urine, so the dose does not change with kidney impairment.
- Used alone it rarely causes low blood sugar; the risk rises when it is combined with a sulfonylurea or insulin.
Side effects to know
A blocked or runny nose and sore throat are the most common. Rarely it can inflame the pancreas: severe, persistent stomach pain that may spread to the back needs urgent care. Joint pain and blistering skin (bullous pemphigoid) have also been reported.
